If you are preparing for a plan review and want to focus on your living situation, you can jump straight to our online intake form.

For many NDIS participants in Blackburn and Bundoora, the dream of living independently or moving into specialized accommodation starts with a single sentence in a planning meeting. However, simply wishing for a change isn’t enough to secure funding. To successfully get housing goals in your NDIS plan, you need a strategic combination of clearly defined aspirations and “Gold Standard” clinical evidence. Without the right goals in place, the NDIA cannot allocate the budget for assessments, support workers, or specialized bricks-and-mortar housing.

At Posity, we act as your clinical advocates. Our Occupational Therapy services are specifically designed to provide the functional data the NDIS requires to approve high-level housing requests.

Step 1: Word Your Goals for Success

The NDIS is “goals-driven.” This means that every dollar in your budget must be linked to a goal you have set. Specifically, when trying to get housing goals in your NDIS plan, you should avoid vague language. Instead, focus on the “functional outcome” of the move.

  • Weak Goal: “I want to move house.”
  • Strong Goal: “I want to explore my housing options so that I can live more independently and build my daily living skills in a safe, supported environment.”

By using words like “explore options” and “build skills,” you trigger the funding for adult OT assessments and services, which are required to determine if you need SIL or SDA.

Step 2: Gathering the Clinical Evidence

Once the goal is in your plan, the NDIS will usually provide a budget for “Exploring Housing Options.” This is where our OTs in Blackburn and Bundoora step in. Specifically, we provide the following evidence to support your housing goals in your NDIS plan:

Functional Capacity Assessments (FCA)

We conduct deep-dive assessments into your “Activities of Daily Living” (ADLs). Specifically, we document why your current home; perhaps a family home in Mitcham or an older apartment in Preston, is no longer suitable. Consequently, this provides the “reasonable and necessary” proof the NDIA needs to fund a move.

SIL Assessments

If your goal involves living with others while receiving 24/7 support, a SIL Assessment is essential. This process goes beyond a standard FCA. Specifically, our Adult OT Assessment and Services provide a detailed report that outlines exactly how many hours of physical assistance or supervision you require daily.

In Victoria, the NDIA looks for evidence of how your disability impacts your ability to perform tasks like personal care, medication management, and overnight safety. By documenting these needs meticulously, we ensure your housing goals in your NDIS plan are backed by the data required to fund a consistent support team. If you are ready to begin this process, you can start your application here.

SDA Assessments

For those with extreme functional impairment or very high support needs, the physical structure of the home itself needs to be specialised. This is where an SDA Assessment comes in. To secure SDA funding, our occupational therapists must prove that standard housing, even with modifications, is not sufficient for your needs.

We assess your requirements against the four NDIS design categories: Improved Liveability, Robust, Fully Accessible, and High Physical Support. Whether you need wider doorways for a power wheelchair or reinforced walls for safety, our clinical evidence serves as the bridge between your housing goals and the specialised bricks and mortar funding needed to move into a purpose-built home. You can complete our intake form to discuss your specific SDA requirements with our team.

Trialing Independent Living Skills

We don’t just write reports; we help you prove you can do it. Specifically, our community occupational therapy team might work with you to practice skills in a community setting. We often utilize Allied Health Assistants to provide high-frequency practice of the skills you’ll need in your new home, such as meal prep or laundry.

Step 3: Aligning Therapy with Housing Transitions

Housing goals aren’t just for adults. Specifically, we start the “preparedness” journey early.

Utilising Telehealth for Rapid Reviews

In 2026, the NDIS often moves quickly once a housing vacancy is found. If you need a quick addendum to a report or a consultation for a new piece of equipment for your home, our telehealth services allow us to provide clinical advice without the delay of travel time. This ensures that your housing goals in your NDIS plan remain supported by up-to-date evidence, even on short notice.

Specifically, we serve families across Melbourne’s east and north who are ready to take the next step in their housing journey.
